New Patient Intake Form
To help us better serve our first-time visitors, we ask each new guest to please fill out our New Patient Intake Form. These may be filled out at our Akron medical marijuana dispensary, or any Botanist location, and we are available to help answer any questions.
We also offer a convenient pre-visit download option which allows guests to fill out the form in advance if they so choose. Simply click the link below to print and fill out the form to bring with you for your first visit.
